Nominate a DoGood-er

Who do you know that shines in the community through volunteering, helping others in need or "doing good?" Nominate them below where indicated, within our categories for possible recognition at our awards event.

Deadline for Entry: August 16, 2024

Please fill in the INTENT FORM
completely with your intent to nominate someone from the Frederick Community before August 9th.

Upon completion of gathering the backup materials, please fill in the NOMINATION FORM
that includes necessary backup requirements by August 16th .

Backup Material Items to Send:
- Nomination letter (preferably on letterhead from the person taking the lead on the nomination)
- Minimum of 2 recommendation letters (preferably on letterhead from people who can corroborate the deeds of the nominee)
- Any backup you'd like to provide (optional backup includes - news stories, flyers, brochures, programs etc.)
- Photos of the nominee in action
- Headshot or recent photograph of nominee
 

Step 1:

If you are intending to nominate someone but don’t have all the backup materials
and the date falls on or before August 9th, use this form.
But then come back by Aug 16th to finalize the nomination with the nomination form.

Step 2:

If you are intending to nominate someone AND have all backup materials, use this form.
If you filled out the intent to nominate form before the 9th but now have your backup for the nominee,
please use this form.
